Service Request
To request a particular service, please complete the form provided below. All details shared on this form will be treated with utmost confidentiality. Please note that if there is a conflict of interest or if the matter falls outside our office’s purview, we may redirect your consultation to the appropriate office or individual. We are committed to connecting you with the most suitable resource to address your needs.
Scheduling and Response Time
All workshop/training requests must be submitted at least three weeks prior to the desired date(s). We aim to respond to all requests within three business days. Workshops are typically conducted Monday - Friday, 9:00 am – 5:00 pm. We understand that not all employees work these hours, therefore, we can work with you to fulfill your request outside of these hours based on our staff’s availability. Please note that we serve the employee (staff and faculty) population. We encourage students to contact the appropriate student service offices in their college and/or the Dean of Students for workshops.
Request Topics
- Consultation, including assessment of potential impacts to modifying campus policies or procedures, and development of strategies to enhance diversity, inclusivity, equity, and belonging within your unit or department.
- Workshops: Interactive sessions focusing on skill-building and practical application.
- Customized Programs: Tailored sessions designed in collaboration with departmental leaders to address unique challenges and goals.
- Colleague Network Group (CNG) membership and topics.
Examples of topics that the Department of Inclusion and Belonging will respectfully decline to address, but will provide appropriate referrals for, include (but are not limited to):
- Employee conduct and disciplinary matters
- Student conduct and disciplinary matters
- Policy 6.4: Prohibited Bias, Discrimination, Harassment, and Sexual and Related Misconduct
