Print Healthcare ID Cards: Endowed Plans

Need ID cards for your endowed plans? Here's how to get them.

Aetna Health Insurance

  • Go to www.aetna.com.

  • Click “Log In/Register”.

    • Already registered? Click “Member Log In” if you have an Aetna Navigator account, sign in using your username and password.

    • Not registered yet? Click “Sign Up Now” if you don’t currently have an online account. If you don't know your Aetna ID number you can use your Social Security number, then follow the instructions to set up your account.

  • When you get to the main page hover over your name in the top right-hand corner of screen.

  • Choose "ID Cards" from the drop-down menu. "View cards for" to select cards for yourself or your dependents. You can print a card for yourself or all members of your family by clicking the printer icon below the card shown.

Don't have access to a printer? Click "Request ID Card" to have Aetna mail a card to your address on file within 7-10 business days, or call Aetna at 1-877-371-2007.


Optum RX Prescription Drug Coverage

Don't have access to a printer? Enter quantity desired (1-9) and OptumRx will mail physical ID cards directly to you.

If you don't have your OptumRx card at time of purchase, you can still obtain your prescription:

  • Let the pharmacist know that OptumRx is the insurer and provide the following information:

    • Employee ID or SSN#
    • BIN:  610011
    • PCN:  IRX
    • RxGroup:  PURCORNEL
  • Pay any applicable copay


MetLife Dental

You don't need to present an ID card to your dentist, but you should let them know that you're enrolled in the MetLife Preferred Dentist Program. Your dentist can verify your coverage by calling 1-877-MET-DDS9.


MetLife Vision

You don't need an ID card to schedule an appointment or receive services from a participating network provider. When visiting a participating vision provider be prepared to provide your Cornell employee ID number.
