Affordable Care Act: Tax Form 1095-C
- The 1095-C Form is used to report health care coverage offered to you. The Affordable Care Act requires employers with at least 50 full-time workers to offer their employees health insurance. Each year those employers must send a statement (Form 1095-C) to all employees eligible for coverage, regardless of whether they participate in the employer's health plan. The form reports information on health coverage offered to employees and dependents enrolled in that coverage.
- You are not required to send a copy of the Form 1095-C to the IRS with your tax return. However, you should store it with the rest of your important tax information, like W-2s, when you do receive it.
- A 1095-C form will be sent by mail to your home address every year, unless you elect to download the form electronically in Workday; see instructions below.
- Online statements require a computer with internet access and Adobe Reader (click here for free download) to view, download, and print the tax document.

Login to Workday

Select "Benefits & Pay"
On the Workday home page, click on the Menu in the upper left-hand corner; then select "Benefits & Pay"

Select "Benefits"
Select "Benefits" in the left sidebar to open the drop-down menu

Select "ACA Forms"
You can view, print, or download a PDF of your 1095C statement.
How to choose to receive an online 1095-C statement
Save paper and energy by electing to download your 1095-C form via Workday.
Navigate to the "View Your 1095C" webpage as described above, then select the "Change 1095-C Printing Election" button at the top of the page. Select the option of your choice (either "Receive both electronic and paper copies of 1095-C, or "Receive electronic copy of 1095-C;" then scroll down to bottom of page and select the orange "OK" button.
After you submit your choice, it will remain valid for all future tax years unless you decide to change your option again in Workday (so this process does not need to be repeated). If you do not choose the online option, you will be provided with a paper copy of your 1095-C. You will still be able to access the electronic version as well.
