Print Healthcare ID Cards: Contract College

Need ID cards for your contract college plans? Here's how to get them.

Empire Plan

 NYSHIP requires that you send a written request for ID cards to the HR Services and Transitions Center (HRSTC) via email at hrservices@cornell.edu.

Include your current home address and the number of cards you are requesting.

Be sure that your home address is current in Workday.

HMO Plans

ID cards must be requested directly from your HMO.

Access NYSHIP online for the telephone number associated with your HMO.

EmblemHealth Dental

ID cards must be requested directly from EmblemHealth by calling (800) 947-0101.
