How To Request A Disability Accommodation
What to do if you need assistance to complete your work due to a temporary or permanent disability.
What To Know
- Disability accommodations can include special equipment such as assistive listening devices or screen readers, worksite and parking accessibility, and flexible work arrangements.
- Qualified disabilities may include conditions such as (but not limited to) impairment of mobility, sight, or hearing; autism, cancer, diabetes, depression, and post-traumatic stress disorder.

What To Do
Contact your manager, HR representative, or the Cornell Office of Civil Rights (COCR)
- Managers will notify the appropriate human resources representative of any requests brought to their attention.

- COCR Accessibility & Accommodations website
- Students need to contact Student Disability Services.
Submit Documentation
Two forms must be submitted to COCR:
- Disability Accommodation Request - Employee Form (pdf)
- Employee Disability Accommodation Request - Health Care Provider Verification Form (pdf) - Must be completed by physician or medical professional. Do not give medical records to your supervisor.
The contents of the forms are confidential and they will not be placed in the employment record file.
What Happens Next
- COCR will review these requests in accordance with the policy, make a determination and then begin exploring reasonable accommodations as appropriate.
- The supervisor and the department are responsible for implementing the recommended accommodation, with assistance from COCR and other campus offices as needed.
- The local HR representative is available to assist with the implementation of the recommended accommodation.
